An assisted living community in Washington state has had conditions put on its license for the third time in three years, reports the Kitsap Sun.

The state’s Department of Social and Health Services also fined the 70-bed Park Vista $1,400 for numerous violations, the newspaper said, including failure to properly monitor residents, update or follow resident service agreements, properly oversee delivery of medications and fully investigate four safety incidents. “All of the violations had been identified by inspectors in 2014 and were either repeated or not corrected,” the newspaper reports.

The community was ordered to hire a nurse consultant to improve its medication management system and train staff members on how to investigate incidents and evaluate residents’ health.
