Addus HomeCare Corp., a provider of home care services as well as hospice and home health, disclosed on Wednesday that it has entered a value-based post-acute care navigation agreement with Presbyterian Health Plan in its New Mexico market.

Commenting on the deal during a call with investors as part of the Stephens Virtual Best Ideas Conference, Addus President and CEO Dirk Allison said such partnerships will enable Addus to help reduce readmissions and emergency room visits and that he hopes there will be other such agreements that “lower the costs associated with a high-cost patient population.”

As part of the Presbyterian Health Plan, Addus will perform pre-discharge evaluations on members using dedicated transition coordinators. In addition, Addus will provide targeted training to equip home care providers with the ability to identify changes in condition that might require urgent intervention. Addus will also assist with coordination and scheduling of medical services and programs needed by members to close known gaps in Healthcare Effectiveness Data and Information Set performance tools.

Addus, which is based in Frisco, TX, currently provides home care services to approximately 44,000 consumers through 212 locations across 22 states.
