Humana and Signify Health, a healthcare analytics firm, are teaming up with a network of community partners in San Antonio, TX, to address the social aspects of health for nearly 800 patients in that community.

The two companies are working with the Alamo Area Community Network to identify Medicare Advantage members who might benefit from health-related social services, such as transportation, food, housing and financial assistance.

The initiative is part of Humana’s Bold Goal program that addresses social determinants of health in underserved communities. Texas Medicare President Bill White told McKnight’s Home Care Daily the partnership in San Antonio could serve as a blueprint for other communities.

“We believe that a holistic approach to health, what we call Human Care, helps our members achieve their best health and helps to bridge the health equity divide,” White said. “ Once we have results from our various markets, we will determine if an optimal, replicable model can be developed for further rollout,” White said.

The partnership is designed to ensure that MA members’ social needs are being met and aims to better understand how those needs impact clinical outcomes.
