In what is being touted as a record-breaking deal, Dallas-based Southwestern Health Resources and Landmark Health and its affiliated medical groups have announced a partnership to provide care to older adults with complex medical needs by bringing interdisciplinary medical care into patients’ homes.
They said it is the largest deal of its kind, based on patient numbers. Southwestern Health coordinates care for more than 700,000 patients, and more than 130,000 people access Landmark’s care.
SWHR — which is the integrated network for the University of Texas Southwestern Medical Center, Texas Health Resources and almost 5,500 physicians and other clinicians across North Texas — is a next-generation affordable care organization that has reduced Medicare expenses by more than $120 million over the past three years, according to the organization.
“This kind of innovative care delivery extends our mission to ensure that people living with chronic conditions have an opportunity to receive the best medical care in the comfort of their homes,” said Sanjay Doddamani, M.D., chief physician executive and chief operating officer at SWHR.
