Six “chronic” care quality measures are to be used for public reporting in the national rollout. These include:

1. Residents with loss of ability in basic daily tasks.

2. Residents with pressure sores, without facility admission profile (FAP) adjustment.

3. Residents with pressure sores, with facility admission profile.

4. Residents with pain.

5. Residents in physical restraints.

6. Residents with infections.

Four “post acute” care quality measures are to be used for public reporting. These include:

1. Short stay residents with delirium, without FAP. 

2. Short stay residents with delirium, with FAP. 

3. Short stay residents with pain. 

4. Short stay residents who walk as well or better.
