Brookdale Senior Living hopes to reduce hospital readmissions by offering Iris Plans‘ advance care planning telehealth services to its residents through a new partnership announced by the companies.

The technology aims to help residents understand their conditions, determine their health goals and create preferences for future care. Iris Plans will distribute the residents’ advance care plans securely to anyone the resident chooses, such as family members and doctors.

Residents will be able to use the healthcare planning services from their communities through Iris Plans’ videoconference capabilities.
